The goal of the District Nursing team is to promote a safe and healthy school environment for all of our students to thrive. We work diligently to support the academic success of our students through the integration of professional nursing skills, health education, and collaboration with our multidisciplinary team, parents, care teams, and community partners. We recognize each students individual needs and acknowledge the importance of working to help them reach their fullest potential.

State Required Screenings/Assessments
Oral Health Screenings
Oral Health and Student Attendance
Oral health is an important factor in student attendance and overall well-being. Students experiencing mouth pain are more likely to miss school, contributing to chronic absenteeism. In 2022, California students missed more than 860,000 school days due to oral health related issues.
Tooth decay is the most common chronic childhood disease, yet it is highly preventable. Good oral health supports children's social-emotional development, helps them focus in school, promotes healthy sleep, and contributes to proper nutrition.
Kindergarten Oral Health Assessment (KOHA)
California law requires a Kindergarten Oral Health Assessment (KOHA) for students enrolled in Transitional Kindergarten (TK), Kindergarten (K), or First Grade if it is their first year in the California public school system.
Parents and caregivers should:
- Take the KOHA form to their child's dental appointment.
- Have the dental provider complete the form.
- Return the completed form to the school as early as possible during the school year and no later than the end of the Kindergarten year.
The assessment only needs to be completed once, in TK, Kindergarten, or First Grade (if it is the student's first year in a California public school).

Exciting News for 2026–2027
We are pleased to share that Dr. Rubin Sorrell II will be providing on-site dental services for students beginning in the 2026–2027 school year. This opportunity is made possible through the efforts of our Student and Community Engagement Coordinator, Gigi Gonzalez.
Additional details about these services will be shared as they become available.
Vision and Hearing Screenings
- Vision: Grades TK, K, 2, 5, 8 and first year in the California public school system
- Color Vision: Grades 1 boys only
- Hearing: Grades TK, K, or 2, 5, and 8, and first year in the California public school system
In addition, students who are being assessed for initial and three-year review for special education services, and students referred by a teacher or parent will also be tested for vision and hearing. Those who do not pass the initial screening will be retested. Parents will be notified and referred for further follow-up only when a problem is identified.
If you do not wish to have hearing and vision screening completed at school for your student, or would like to opt out of the oral health screening from your child'd dentist, you must submit a waiver, which are both located under the form section below. For your convenience, a list of health clinics in San Mateo County is provided to assist you in scheduling a health exam and/or a vaccine appointment.

Chronic Conditions
When a chronic medical condition presents it is the role of the District Nurse to work in collaboration with the student’s healthcare providers, healthcare teams, and a variety of specialists to ensure that an appropriate plan of care is provided by the student’s physician and implemented within their school setting. Once a set plan of care is in the place the district nurse then educates the school interdisciplinary team on the plan and provides adequate training to staff members working with the individualized students to promote safety and security within our school communities.
**If your student has a preexisting action plan in place, or will need medication on campus, new forms will need to be provided each new school year and submitted to the district nurse. Please make sure that you have the physician create an updated care plan, as well as complete a medication authorization form so that we can provide care to your students in alignment with the doctors orders. Additionally, if your child will be carrying medication on campus a consent to carry form must be completed by a parent/guardian and signed by the student and submitted with the medication authorization form (completed, signed, & stamped by a licensed healthcare provider).
WITHOUT THESE COMPETED FORMS NO PRESCRIPTION OR OVER THE COUNTER MEDICATION OR SUPPLEMENTS WILL BE ALLOWED ON CAMPUS.
